Mental Health Discharge Liaison Nurse

What we do

The Mental Health and Addictions Program currently offers the services of a discharge liaison nurse who provides clinical care to St. John’s and the surrounding areas for patients discharged from psychiatric acute care units at the Health Sciences Centre and the Waterford Hospital.

The role of this discharge liaison nurse is to facilitate the transition of patients from hospital following discharge and help individuals access follow-up mental health services and supports in the community. Where possible, the discharge liaison nurse meets with patients in hospital during the discharge process and, subsequently, provides a full range of clinical service while linking patients to their community. This specialized nursing follow-up is most often provided in the community within the context of home, office and/or community resource visits.
