Geriatric Intake Coordinator

What we do

This is a specialized geriatric psychiatry nursing position which is primarily responsible for the comprehensive assessment of people over the age of 65 who require mental health services. The intake coordinator also provides a single point of access to programs and services offered by the geriatric psychiatry program. This position provides a vital link for integrating clinical efficiency principles with geriatric psychiatry services.

How to access this service

The geriatric intake coordinator receives referrals from health professionals to either outpatient or inpatient geriatric psychiatry services. These health professionals can submit a completed geriatric psychiatry and medicine referral form.
