Assertive Community Treatment Team

What we do

The Assertive Community Treatment Team (ACTT) assists adults who have severe and persistent mental illness to live in the community, to better manage their symptoms, achieve personal goals and maintain optimism. ACTT involves treatment, rehabilitation and support services.

Treatment services include psychopharmacological, individual supportive, crisis intervention and psychiatric hospitalization, if necessary.

Rehabilitation services include assisting clients with structuring time and employment, activities of daily living and social and interpersonal relationships and the use of leisure time.

ACTT also provides support services which involve assisting clients with medical and dental appointments, financial and housing issues, money management, transportation, legal services and collaboration with families and assistance to clients with children.

Who we see

All ACTT clients have a diagnosis of either schizophrenia, schizoaffective disorder or bipolar disorder.

How to access this service

Clients can be referred, using an ACTT referral form (PDF), by health-care professionals, community or government agencies, family members or by self-referral.

Individuals referred will be scheduled for an intake assessment to determine if the ACTT program is the appropriate service.

Hours of operation

Our hours of operation are 8 a.m. to 8 p.m., Monday to Friday
10 a.m. to 6 p.m., weekends and statutory holidays
